Healthcare Transformation: Value Based Care
For decades the US Healthcare system has been rooted in a Fee For Service model where the focus has been on diagnosis and procedures… not outcomes or value. There has been pressure on the Fee For Service model for years but that pressure has become more pronounced in the form of CMS’ “Better, Smarter, Healthier” initiative that set milestones for shifting Fee For Service payments to Value Based Care models and also set goals to accelerate all Medicare payments to Value Based Care models. Other payers (e.g., Medicaid, Commercial) are also supporting this transformation.
One particular Alternative Payment Model (APM) that healthcare providers have adopted is the Patient-Centered Medical Home (PCMH). The PCMH model promotes more coordination among clinicians as well as more use of population health management, Health Information Technology (Health IT), continuous improvement, and patient self-management. There are several national bodies that provide PCMH recognition standards, including NCQA.
